Instructions to download, edit, and save
- You must have Adobe Acrobat Reader version 5.0 or higher
- Once the file is downloaded and saved to your computer, fill in the form by clicking on a line and typing the necessary information.
- To save the document for your records (highly recommended), please follow these instructions:
- File -> Print
- Change (if necessary) the printer’s name to: Microsoft Office Document Image Writer
- Click “print”
- It will then ask you to save the document, which is where you’ll have the opportunity to save the file to the appropriate folder on your computer
- Click “save”
- You will then be able to open the document and view it, but will not be able to make changes to the document
- You may return each form to the SGA office located in the Memorial Student Center, Room 2W29B
Please note that falsifying any information will greatly decrease the credibility of your application and furthermore may decrease the chances of receiving funding this semester as well as future semesters.
